As governed by the laws of the professional regulatory bodies and the Province of Quebec, a donor’s consent is needed for the collection, use and disclosure of personal information. Des Collines Health Foundation is committed to protecting the privacy and security of your personal information,
- What is personal and personal health information?
Personal information includes any identifying information about you. Des Collines Health Foundation collects personal information from you such as your name, date of birth, address, email and phone contact information.
- Why does Des Collines Health Foundation need to collect, use and disclose your personal or personal health information?
To provide receipts for donations and to give regular updates via our e-news should you desire.
- Giving your consent
Your consent must be freely given. You need to understand the purposes for which Des Collines Health Foundation will collect, use or disclose your personal information before you give your consent and understand that you are able to withhold consent or may withdraw your consent, in whole or in part, after it has been given.
Des Collines Health Foundation will make all reasonable efforts to ensure that the purposes for which the information will be used are identified to you so that you can provide knowledgeable consent.
Personal information will be protected and never released to a third party without a donor’s express consent. Storage, retention and destruction of personal information will comply with existing Quebec provincial legislation.
- Withdrawing your consent
You have the right to withdraw your consent to the collection, use or disclosure of personal or personal health information in whole or in part, at any time upon providing reasonable written notice to the director of Operations of the Des Collines Health Foundation. If you withdraw your consent it is not retroactive, and does not apply to personal information already collected, used or disclosed by the Des Collines Health Foundation. - Safety and Accuracy of Personal or Personal Health Information
We strive to keep your personal information as accurate, complete and up-to-date as is necessary for the purposes for which it is to be used.
Des Collines Health Foundation has established a variety of safeguards to protect personal and personal health information in its care, including organizational (for example: training personnel about privacy obligations), physical (for example: locking filing cabinets, central filing of active charts) and technological safeguards (for example: virus protection, computer backup, password entry to computer system).
Des Collines Health Foundation will retain your personal information for the time necessary to fulfill the purposes for which it was collected and to comply with its legal obligations and to meet regulatory requirements.

- Donation history (number of donations and sectors/projects to which these donations are given);
- Registration history (number of registrations and events for which these registrations are made);
- Information for official receipts or invoices.
Financial Information
You will be asked for certain financial information if you make a credit card payment. This information is processed through a secure and PCI-compliant gateway to process the credit card payment.
Changes to This Policy
The Des Collines Health Foundation reserves the right to modify this policy as needed, for example to adapt it to legislative and regulatory changes. Consequently, we advise you to consult the Policy regularly to validate its content. In the event of modification, the modified version will be available on the website.

a) The Des Collines Health Foundation is responsible for personal information in its custody or control.

The Des Collines Health Foundation collects and uses personal information for the following purposes:

b) Administer one-time or programmed donations or payments;
c) Generate statistics and summary reports for internal and external use, the latter without reference to personal information;
d) Communicate directly with users for charitable purposes or other forms of communication in compliance with Canada’s Anti-Spam Legislation (CASL);
e) Provide support and guidance to customers.
Consent for the Use of Your Personal Information
a) The Des Collines Health Foundation collects personal information about you only when you voluntarily provide it to us, or only with your consent, as required by PIPEDA or any other applicable law;
