Our mission is to raise funds to have an impact on the well-being and health of the Des Collines community in partnership with the Outaouais Integrated Health and Social Services Center.
WE SUPPORT
TWO LONG TERM CARE FACILITIES
○ CHSLD des Collines, Masham (24 beds)
○ long term care wing in the long term care stay of the Wakefield Hospital (10 beds)
where residents receive the care they need while staying close to their community, family and friends.
FOUR COMMUNITY HEALTH CENTRES (CLSC)
○ Cantley ○ Chelsea
○ Masham ○ Val des Monts
they provide medical services that do not require a hospital visit – these services are essential to our physical and mental health as individuals and as a community.
WAKEFIELD MEMORIAL HOSPITAL
Just last year, the hospital had some 13,000 patient visits – that’s about 42 patients each and every day, seven days a week.
